Community Care TASMANIA is your local not-for-profit home care provider. We offer a full range of home-based aged care services. Our aim is to assist and support people to remain living in their own homes in a safe, friendly, comfortable way. Our friendly and expert staff are always available.

During our 26 years of service we have evolved from roots in our migrant communities into a unique small scale organisation that prioritises people not profit.
We deliver exceptional home care services to all Tasmanians whilst maintaining our expertise in serving the CALD (Culturally and Linguistically Diverse), community. We also welcome LGBTI clients.
Our clients range from aged care through to NDIS clients and those under 65 with individual support programs, (ISP) . They include people with a range of support needs from Motor Neurone Disease, (MND) to veterans’ care.

Community Care TASMANIA is your local home care provider of home based aged care support and packages. We operate under the following government and non government programs.
On 1 July 2015 the Commonwealth HACC program began to be phased out. All new consumers are now offered Commonwealth Home Support Program support or a home care package. However Community Care TASMANIA will continue to deliver existing HACC programs to its current consumers who choose to remain under this scheme until 1 July 2018. Should your care needs increase we can support your application for a home care package.
We are also accredited under the Tasmanian Government’s Home and Community Care (HACC), Program and Individual Support Program (ISP). These provide funding for services to support younger people (less than 65 years) and their carers. They are designed to assist those whose capacity for independent living is at risk through mental or physical disability or accident and/or who are at risk of premature or inappropriate admission to long term residential care.

What does Consumer Directed Care, (CDC), mean for those accessing home care?
Consumer Directed Care (CDC), is a term that is used to describe the new approach to providing home care packages.
It means that you are given an annual budget – comprised of the government funding you will receive as well as the contribution you may be asked to make – and the right to use this budget to purchase the aged care services you need.
Under CDC the money from your Home Care Package is yours, to be used to access the services you want, from the support worker or organisation you choose.
For the first time, it enables you to shop around for the types of services you want, and choose who you would like to provide those services. Naturally we hope you choose Community Care TASMANIA. We are confident that we can ensure your funding goes further.

Established In 2014, this fee paying service is available to anybody including those not eligible for government support. Whatever your age and background you can access support services just when you need them.
