Project Coordinator

About us

Due to organisational growth we have an exciting opportunity for a Project Coordinator in our Home Care Packages (HCP) team. This key role will be contribute to optimising Home Care Packages client numbers and funds utilisation for HCP Program.

Specifically to:

Develop plan organising and delivery of a new health program for HCP clients that will address unique needs of HCP clients. A fundamental part of this exciting new position is to; understand clients’ needs and then to develop, and market those unique and special programs in order to address the needs you have uncovered.

In order for you to do this you will need planning skills, investigative skills, sales and marketing skills, great communication and listening skills along with planning, liaison and implementing and finally project management skills. A key skill component will be fabulous IT skills and the ability to develop online platform programs of interest to the HCP recipients.

About the Role

  • You will be part of assisting to create some new programs to support HCP clients.
  • You will need exceptional communication skills and the ability to promote your designed program to HCP clients
  • You will need to be self-motivated, committed and have project management skills and excel with outcomes.

Essential Criteria

  • Demonstrated knowledge and understanding of the Aged Care industry and the various funding programs aligned with all services, or a willingness to learn
  • Exceptional customer service and problem-solving skills.
  • Strong negotiation skills
  • Excellent presentation skills especially to community groups
  • Experience, knowledge and understanding of issues in relation to frail elderly clients, people with dementia, including those from a culturally and linguistically diverse (CALD background ) and implications for clients , primary carers, families and community agencies.
  • Reliability, flexibility and commitment to high work standards.
  • Demonstrated ability to plan, implement and organise services.
  • Exceptional written communication skills and records management skills
  • Ability to self-motivated, work autonomously and interdependently in a dynamic and fast paced work environment.
  • A current Working with Children Check or willingness to obtain and maintain current registration
  • National Police Check and commitment to maintain
  • A current unrestricted Driver's License
  • Intermediate computer and MS office skills
  • Provide two work related references

Desirable Criteria

  • Recent experience working as a Case Manager/Coordinator or advocate in Aged Care, or understand of the sector or commitment to achieve this
  • Experience in the implementation of social support groups.
  • Program development experience
  • Sales and marketing experience
  • Delivery of programs experience
  • Project management experience

Casual Support Workers Wanted in Port Sorell & Ulverstone!

Are you passionate about providing care, making people feel valued and assisting someone to maintain their independence?

Do you want a few hours of work providing high quality support to aged and vulnerable clients living in their own home?

We’ve been asked to provide care to people in the Port Sorell & Ulverstone area so we’re looking to hire now.

What we are looking for:

Someone who is available and willing to accept any casual hours offered across our 7-day roster as clients from the area come onboard with us

  • Someone who has experience working in the community sector and has a Certificate 3 in Individual Support, Aged Care or Disability Services.
  • Ambitious people who aren't afraid to work hard and make a positive change!
  • Someone who knows they can deliver quality one-on-one home care to our clients who have a broad range of support needs.
  • Someone who is willing to work flexible hours (maybe even evenings and weekends).
  • As a Support Worker with Community Care TASMANIA you will be required to provide support including:
  • Personal Care: showering, dressing, grooming and possibly medication prompting
  • Domestic Assistance: meal preparation, cooking, household cleaning and laundry
  • Social Support/Transportation: transport to medical appointments, grocery shopping and social activities
  • Skills development support for disability participants: helping build capacity with life skills

It is essential that you have:

  • Certificate 3 Individual Support (Disability) or equivalent
  • Satisfactory National Police Check
  • Tasmanian Working with Vulnerable People Registration
  • Drivers Licence with a registered, reliable and suitably insured car
  • For non-citizens, a suitable visa to work and live in Australia
