Community Care TASMANIA is your local not-for-profit home care provider. We are proud to be a truly Tasmanian service and we have been recognised in both national and state awards this year. Community Care TASMANIA is a small scale, local not-for-profit service delivering the highest quality home-based care services.

We know our community and our focus is entirely on home-based care. We know how best to help you through the maze of aged and disability home care. Our work has been recognised in national and state awards during 2017

We understand the care needs of all Tasmanians. Our established links with migrant communities also means we are the best qualified aged and disability care service whatever your background or wherever you live in Tasmania.

Community Care TASMANIA is fully accredited to deliver a comprehensive range of federal and state government funded home care services. The 2016 Australian Government Aged Care Quality Review gave Community Care TASMANIA the highest rating in all 18 audited criteria. Your needs will be met through either the Commonwealth Home Support Programme, (CHSP), or the more extensive home care packages

In September 2017 we also received a Better Practice award from the National Aged Care Quality Agency

We can also provide home-based Palliative Care.

Our services also include home care support for people under 65 who may otherwise be prematurely placed in residential care.

Need direct support around the house or beyond?  We provide help when you need it, for as long as you need it, to anybody who needs it through our self funded Dial an Angel service. 

We know the important role that maintaining and strengthening social connections plays in quality aged care. That is why we have established the dynamic creative connections, community connections and community visitor programs.

Our fees are very competitive and completely open and transparent. There are no hidden costs to you, and definitely no surprise add-ons. Meeting your needs is our priority. We are always happy to discuss how client contributions can best reflect your personal circumstances.

If you need further help and/or advice you can now contact the National Aged Care Advocacy Program.

This service provide free, confidential and independent advocacy support to older people (including their families and representatives) who are receiving, or seeking to receive, Australian Government funded aged care services.

Government Funded Home Care Packages and Home Support Programs

Our staff are highly qualified and respected because they are friendly, approachable and efficient.

As your local home care provider, Community Care TASMANIA offers you a full range of commonwealth and state government aged care support services in your home. We are always help you through the assessment process.

Once you have been assessed and your care needs defined,  we will work with you to design and deliver your services..These range from basic care Community Home Support Program through to the more comprehensive home care packages as well as home based palliative care.

We are different because we are small scale, Tasmanian based and have a long history of delivering local home care services to Tasmanians of all backgrounds and cultures.

Together with our small scale and people-centred approach this means we are best placed to provide the personalised and flexible care that you or your loved ones need.

Self Funded Home Care and Support

A helping hand is always welcome and always available by purchasing any of our Dial an Angel services.

Anyone in need can use this service.You can purchase the service you need just when you need it. It doesn’t matter if you are old or young, working or not or whatever your background, our angels are there to help you.

For a one-off need or for on-going help, just let us know and we will find a solution.

Maybe you need a cleaner, a gardener, a nurse, transport to get out and about to do the shopping or visit family? How about someone to do odd jobs around the home?

Community and Wellbeing Programs

We understand that supporting people to live well is about more than good care at home.

Quality local home care is often about maintaining social and creative contacts and learning new skills. That is why we have established our unique Creative Connections and Community Connections programs. These enable clients to come together to create artworks, undertake social outings, make friends and learn new skills.

Community Care TASMANIA volunteers also regularly visit clients in residential care. This can bring great rewards for both clients and volunteers.
