Support At Home Care Manager

The Care Manager is accountable for managing the delivery of high-quality, person-centred care to participants funded under Support @ Home, ensuring that services are consistent with the relevant legislation, quality framework and funding guidelines through clear communication, system administration and linking individual work to funding outputs to ensure appropriate planning of services is consistent for Participants across Tasmania. 

Care Managers are essential to the provision of a high standard of care and support to Participants at Community Care TASMANIA and to maintaining strong relationships with Participants, families, and service providers. This is an office-based role to ensure efficient collaboration within the team.

In home Aged Care Support Workers – across Tasmania – Remote

In this role you will work closely with participants to assist them to continue to live in their own home. You will be part of a growing and exciting team and supported with enthusiastic training and administrative support. You will be part of team that cares and puts its clients first. The dynamic nature of our services, coupled with the chance to work with multiple clients, guarantees regular opportunities for you to contribute significantly.

Key locations we are actively recruiting in are Launceston & surrounds, Devonport, Burnie, St Helens and Hobart areas, but all applications will be considered.
