Community Care TASMANIA's innovative and successful Community Connections Program has been acknowledged in the Natiional Aged Care Quality Agency Better Practice Awards for 2017. We are the only Tasmanian Agency to recognised in this year's Better Practice Awards.
The Agency highlighted the tremendous work done in researching and identifying the need for social and aged care support in the local Bhutanese community in the first instance as well as the great success that Community Connections has achieved.
The program provides a weekly meeting point in Mowbray and the opportunity to participate in a series of excursions, information and social activities all of which aim to promote social inclusion, a sense of belonging and greater awareness of what services and facilities are available in around Launceston.
