Getting assessed for Home Care - How we can Help

As part of the move to Consumer Directed Care, (CDC) a new system for assessing individual need for government-subsidised home care and aged care services also came into force.

When you talk to My Aged Care staff they will ask you a series of questions to determine whether your required level of support is either CHSP support or a care package. In the first instance you will be referred to the Regional Assessment Service, (RAS). If it is clear that your needs are very high you may be referred directly to the Aged Care Assessment Team, (ACAT).

The RAS will assess your actual level of need. This could range from lower level care services through the CHSP, other services such as physiotherapy through to a comprehensive care package. In the latter case ACAT will assess what level of package you will need.

Community Care TASMANIA can help you through this process of getting assessed for your required level of home-based support. Contact us to speak to one of our professional and friendly staff.

The federal government has selected a number of organisations to administer assessment services. Community Care TASMANIA was successful in being appointed as a sub-contractor and when requested by the major provider, Serendipity WA Pty. Ltd., can provide Regional Assessment Services in Tasmania. This allows us to utilise our unique expertise in assessing people from CALD, (Culturally and Linguistically Diverse) backgrounds.

If you are a CALD consumer, you can request that your assessment be made by an approved CALD assessor and ask for Community Care TASMANIA to be your care provider.
